SkyScribe is an AI Clinical Operating System that provides drafting, structured documentation, and clinical decision-support to qualified, registered medical practitioners across specialties.

From the history, examination findings, investigations, and clinical observations a clinician enters or dictates, the platform applies structured reasoning to help draft consultation notes, admission and discharge summaries, operation and procedure notes, prescriptions, referrals, and other clinical documents, and to surface differential considerations, suggested investigations, and management prompts.

SkyScribe does not examine patients, does not perform its own tests or measurements, and does not replace the knowledge, judgment, or responsibility of the treating clinician.

Every AI output in SkyScribe is a DRAFT. Nothing is entered into the patient record, prescribed, ordered, or issued until the responsible clinician reviews, edits, and explicitly approves it. Any diagnostic, differential, or management suggestion is a supplementary prompt to support the clinician's own reasoning — it is not a diagnosis, a prescription, a clinical recommendation, or a directive, and the clinician retains complete discretion to accept, modify, or disregard it.

Deterministic safety and calculation features (clinical scores, drug allergy / interaction / renal-dose checks, critical-value flags, dosing arithmetic) are computed by tested, rule-based logic to assist the clinician; they do not remove the clinician's duty to independently verify every dose, order, and decision. Seed reference data is provided for development and demonstration; a production deployment must use a licensed, validated clinical data source.

SkyScribe is a clinical decision-support and documentation tool. It is not a medical device, does not provide medical advice to patients, and creates no doctor–patient relationship between SkyScribe and any patient. It is not for use in an emergency; in an emergency, follow local emergency protocols.

Final diagnosis, prescribing, ordering, documentation, and all clinical management decisions remain at all times under the supervision and sole responsibility of the qualified, registered medical practitioner.
